Firstly download ATM card application from the above link and take a printout and start to fill from top of the application. You have to fill the application form only in CAPITAL LETTERS.
- Firstly, write TMB branch name in the respective place
- Then, write your Full name, Date of birth in DD/MM/YYYY format and Mother name in the respective place.
- Then write your communication address, mobile number, email id and Aadhar number in the space provided.
- Now specify your account type whether CA/OD/SB
- Then write your 15 digit account number in the respective field.
- Finally, write date of the application and then put your Signature
- If your account is joint account then both the applicant must sing in the application for availing ATM card
Submit your filled in application to your respective branch. The branch officials will check whether your details are correctly written in the application and proceed for card processing.
