How to fill BOB Debit Card application form?

The following are the step by step procedure to fill Bank of Baroda debit card application.

• Firstly download debit card application from the above link and take print out. Then start to fill from the top of the application. You are advised to write your bank account details only in CAPITAL LETTERS.
• Write your Account type and Account Number in the space provided. Also, write your 16-digit ATM card number if you already have.
• Write your Name as per your bank record, Date of Birth, Gender and also write the name to be printed on the debit card.
• Then write your residential address and Office address details with correct pin code in the respective places.
• Then tick the address where you want to receive your debit card.
• Read the terms and conditions carefully and finally put your signature in the respective place.
• If your account is joint account, all the account holders have to put your signature in the signature column.

Finally, check the application form whether you have written your account details correctly and then submit the form to Bank of Baroda branch.
